New Patient Checklist

Below is a checklist of items and forms that will help prepare you for your upcoming appointment and make your visit as productive as possible.

  • Your most current insurance card
  • A form of identification i.e. driver’s license, passport
  • Method of payment for your co-payment, if any
  • Medical Records. If your relevant records are not already at our clinic, please obtain them and bring them with you
  • Previous X-rays, CT or MRI scans. Actual films or CD is required. Reports alone are not sufficient
  • Results of previous testing or treatment (EMGs, physical therapy, etc.)
  • Bring any corsets, braces or other supports prescribed for you

To expedite your appointment, you may download and fill out the following forms prior to your appointment. If you are unable to do this, please register 15 minutes before your scheduled appointment to complete the paper work.

New Patient Visit

The goal of this visit is to adequately assess your condition and create a plan for treatment. To be effective, previous spinal evaluation and treatment information is needed. Please ensure that all previous medical records are sent to Dr. Sun prior to your initial visit or hand carried to your appointment. We ask that you request all the necessary pre-authorizations prior to your visit.

Please use the New Patient Checklist as guide for items to bring to your appointment. During the initial appointment:

  • Dr. Sun may discuss your medical history, review records, and perform a physical examination.
  • You will receive an explanation of the diagnosis and recommendations for ongoing care that may include non-operative and operative treatments.
  • Additional diagnostic testing (MRI, CT, etc.) may be ordered.
  • Clinic staff will provide instructions for follow-up procedures or appointments.

Allow for a minimum of two hours in your schedule. While this amount of time is not standard, we want you to be aware that it may, at times, be necessary. You will need time to complete paperwork and sometimes Dr. Sun may get behind schedule. (The complexity of patients' needs or emergency situations may necessitate longer appointments. We apologize in advance for any delays these changes may make in scheduled appointments). We look forward to serving you!
